 マンモグラフィ検診により早期癌のみならず乳癌と紛らわしい良性病変が発見される機会が増えている。放射状瘢痕Radial scarは、そのような良性病変の代表格である。放射状瘢痕の過剰診断を避けるため、その病理像のスペクトラムについて習熟することが重要である。というのは、放射状瘢痕は瘢痕化による単純な小葉、乳管の変形だけではなく、他の良性乳腺病変同様、腺症、乳管内増殖性病変、乳頭腫などのさまざまな上皮増殖を示しうるからである。放射状瘢痕を正しく診断し、将来の乳癌リスクを評価するためには、浸潤癌と腺症との区別や、乳腺の上皮増殖のさまざまなパターンに関する知識が必要である。

 Introduction of mammographic screening has led to the discovery of not only small breast cancers but also benign lesions to be differentiated from malignancies. Radial scar is a representative of such mimickers of breast cancers in the mammographic era. To avoid overtreatment of radial scar, it is crucial to know its histopathological spectrum. The morphological hallmark of radial scar is a central fibroelastosis surrounded by radiating benign epithelial elements. As with other benign epithelial lesions of the breast, various proliferative lesions, such as adenosis, papilloma and intraductal proliferative lesions may develop in the epithelial structures of a radial scar. The epithelial proliferation of the breast is basically classified into two patterns, that is, outward (to the lumen) and inward (to the stroma). The former is further divided into papillary and non-papillary (intraductal proliferative lesion). Recognition of these principal patterns is important because the type of intraductal proliferation appears to be value in making differential diagnosis and predicting the general risk for development of breast carcinoma.
