乳房超音波診断の動向

 「乳がん検診における超音波検査の有効性を検証するための比較試験」
J-STARTが終了した。超音波併用群では高い感度を示し、中間期癌症例は半減し
たことが報告され、乳房超音波の併用検診が現実味を帯びるようになってきた。今
後は、特異度を下げるための総合判定基準の作成が急務となってきている。一方、
技術革新も進み、Automated Whole Breast Ultrasoundも開発され、検診への応
用が待たれるところとなっている。日本が開発したエラストグラフィは、乳房の超音
波診断には欠かせないツールとなり、全世界で利用されるようになった。その他
Photoacoustic mammographyの開発状況ならびに将来の展望についても述べる。

 The Japan Strategic Anti-cancer Randomized Trial (J-START) to assess the
efficacy of adjunctive ultrasound in screening for breast cancer in Japanese
women aged 40–49 years, has just finished. It revealed that the intervention
group who had breast ultrasounds could have high sensitivity and decrease
the number of interval cancers. In the next step,we must create comprehensive
criteria to improve the specificity, as soon as possible.
 On the other hand, progressive technology innovation ─ the Automated
Whole Breast Ultrasound, has also been developed, and we await its
application to medical examinations. Elastography developed in Japan has
become a tool indispensable to ultrasonic diagnostic breast treatment, and is
used worldwide. I also discussed the development status and future prospects
of photoacoustic mammography.
